On Friday, the U.S. Senate confirmed Mark Cruz as the new director of the Indian Health Service (IHS), a vital agency that oversees healthcare for approximately 2.8 million Native American and Alaska Native individuals. Cruz, a citizen of the Klamath Tribes, will serve a four-year term, stepping into a position that has remained vacant since late 2025.

Leadership Challenges in Native Health Care

Cruz’s confirmation came via a narrow 51-47 Senate vote, among more than 70 nominations from President Trump, just before the Senate planned to recess for August. During his confirmation hearing, Cruz emphasized his commitment to improving health outcomes in Indian Country and underscored the importance of his personal connections to the communities he serves. “My family, my friends and my tribal community will live with the consequences of the work I do for decades to come,” Cruz remarked to the Senate Indian Affairs Committee.

In addition to his new role at IHS, Cruz will continue his work as a senior adviser on tribal health to Health Secretary Robert F. Kennedy Jr. Previously, he led the Office of Indian Energy and Economic Development within the U.S. Bureau of Indian Affairs during President Trump’s first administration.

The Indian Health Service is responsible for operating 21 hospitals and 78 health centers across the country as part of the federal government’s legal obligation to provide healthcare services to Native Americans. Reports indicate that approximately one-third of these facilities are in poor physical condition, with an average age of around 40 years, according to a 2023 assessment by the U.S. Government Accountability Office.

Cruz faces numerous challenges, including persistent high vacancy rates among healthcare providers and a significant backlog of facility replacements that has accumulated over 30 years. He has promised to address these issues, enhance the resilience of tribal health programs against federal budget uncertainties, and seek funding solutions for overhead costs incurred by tribes managing their own healthcare systems.

The National Congress of American Indians has expressed optimism regarding Cruz’s appointment, calling it a “critical time” for Native health. Executive Director Larry Wright Jr. congratulated Cruz and highlighted the importance of upholding the federal government’s trust and treaty obligations. Similarly, the National Indian Health Board voiced support for his confirmation, noting that meeting the healthcare needs of tribal nations would require an estimated $73 billion—while Congress allocated just over $8 billion for the IHS in the fiscal year 2026.

Cruz’s leadership at the Indian Health Service comes at a crucial juncture, as the agency grapples with longstanding issues in healthcare delivery for Native American populations. His ability to navigate these challenges will be closely monitored, as improvements in health outcomes are essential for enhancing the quality of life for millions in tribal communities.
